A log-in and password is not required to register a new legal entity. However, all other on-line forms submission requests require a user-name and password. Existing providers will receive a notification email requesting they re-apply to renew a regulatory instrument. This email will include log-in and password details for on-line forms. If a provider requires a log-in and password for an on-line form prior to their next notification for renewing a regulatory instrument, they should contact HealthCERT requesting a user-name and password by emailing certification@moh.govt.nz or call 0800113813. This username and password will remain valid for on-going access to on-line forms. Note that existing user-names and passwords issued for prior certification applications are not valid for new on-line forms.
